Student Placement Pre Course Form

Before starting your programme of study we need you to understand how your placements will be organised.  We would therefore request that you fully read the Organising Practice Learning Experiences handbook (click here) before completing the appropriate online questionnaire.

Once you have read the handbook, please click on the relevant link below relating to your programme of study to complete the online questionnaire.

For Operating Department Practice (click here)

 

We will use this information to allocate you to a practice learning area appropriate to your programme and, where possible, your personal requirements.

You will find out which Trust you have been allocated to within the first week of commencing on your programme.

Please submit your form by Monday 13th August 2018.

If there are any changes after submitting your form you must inform Helen Swinburn; Student Placement Manager immediately by clicking here
