We have a short supporting documents guide here.

Some examples of suitable documents would be:

  • A letter from a GP/hospital/Mental Health Professional
  • Educational Psychologist report
  • A patient summary
  • A diagnosis letter
  • Student Finance England (SFE) Medical Evidence Form
  • Referral letters for ADHD/autism testing

However, we encourage all students to register with us using any documents they have access to which outlines a condition. If we require further information, we will contact you.

If you do not have a full psychological assessment for dyslexia, we will be able to offer you an online dyslexia self-assessment screener.

To start this process, please complete our Accessibility Support Form and upload any relevant documents you already have.
