In consultation with Durham/Dales CCG, the research team (The Northern Veterans and Military Families Research Hub) engaged in a pilot project in 2019 to implement a social prescribing model for veterans and military families in four GP practices. A Veterans Gateway Iframe specifically for the GP primary care IT system is embedded and this can track the search history of GP practices using the web application.
This work is progressing and we are working with North East and North West CCG regions to expand this work.
